• If you are a Healthcare Provider, please direct your patients to request their own records at www.cpspain.com. This site is not for continuing care requests from Providers.
• If your request is an audit, please call (972) 210-3469. Do not submit your request here.

What You Need to Get Started

1. A valid medical records release authorization signed by the patient
2. A credit card to make payment

Please have these items handy before you start!

Important - Please Read!

• No billing records are available - only medical records. Do not request them.
• No affidavits, certifications, or notarizations are available. Do not request them.
• No radiology images are available. Do not request them.
• We provide the patient’s entire medical record, regardless of Dates of Service required.
• To ensure patient security, we do not fax or mail paper records. We only fax a notification to go online to access the requested records securely.
• THERE ARE NO REFUNDS FOR ANY REASON WHATSOEVER.
• We only process third party requests submitted through this site.
