This ordering site is for use by patients only.
NOTE:
To make ordering easier, this site is smartphone compatible.
Who Can Order Patient Records Online?

Patients 18 years of age or older should place their own request.

Parents/guardians with legal authority
and who are named in the patient
chart should order for a minor child.

A person with documented legal authority to
make healthcare/medical treatment decisions
for a patient of any age (healthcare conservator,
medical/healthcare POA, legal guardian, executor/administrator
of estate) can order for a patient.
NOTE: Financial POA is not sufficient.
Please have these items handy before you start!

The patient's valid driver's license or other valid
government-issued ID.

If you are not the patient, you will need your valid ID as well.

A credit card, if applicable.

If you have the documented legal authority to make
healthcare/medical treatment decisions for a patient,
you must also upload official, currently valid
documentation of this authority
(Durable POA for Healthcare, Medical POA, Advanced Directive for
patient and designated caregiver; Death Certificate for
deceased patient).
To get started, select one of the four options below.
A separate
order must be placed for each item you wish to request.
All Medical Records

$25 flat fee per request.
There are no refunds.

Allow up to 48 hours for completion.

ALL available Medical Records
(including Imaging Reports) are provided.
You will not be asked to specify types of records, physicians,
or dates.

You will be texted or emailed when your records are ready to
access online.

Records will be made available
online only.
We do not fax, email, or mail records.
Recent Medical Records (Past 2 Years)

Free of charge.
-
Allow up to 7-10 business days, per state
statute, for completion.

THE PAST 2 YEARS TO DATE of available
Medical Records (including Imaging Reports)
are provided.
You will not be asked to specify types of records, physicians, or dates.

You will be texted or emailed when your records are ready
to access online.

Records will be made available
online only.
We do not fax, email, or mail records.
Radiology Imaging Link - Shareable
-
$15.00 flat fee for a secure weblink to your radiology imaging.
There are no refunds.

Allow up to 48 hours for completion.

ALL available radiology images
(excluding Imaging Reports) are provided.

You will be texted or emailed a link to your imaging.
-
Share the secure link with another healthcare provider to
view/download your imaging.
You can also select and view your images from your image
library and/or download the images yourself.
Imaging CD by Mail

$38 per requested CD copy for all
available imaging.
There are no refunds.

Your CD will be prepared by Carrell Clinic's Radiology staff
and mailed to your home address only.

CD pick-up is not available.
-
Allow up to 15 business days, per
state statute, for CD preparation plus
US Postal Service mail delivery time.

You will be texted or emailed when your Imaging CD has
been mailed.
Delivery time by the US Postal Service varies and is not the
responsibility of Carrell Clinic.

For the status of your imaging CD request or issues
with an imaging CD you have received by mail,
call Radiology directly at
